Screening and stratification of diabetic nephropathy are based on the measurement of albumin in urines. In diabetic subjects, the early detection of microalbuminuria optimizes the opportunities for prevention and treatment of diabetic nephropathy. Even if albuminuria has not shown to be and independent risk factor, its wide use as a cheap and easily available cardiovascular and renal disease marker is largely documented and justified in the diabetic population. Whether early detection of microalbuminuria might identify subjects at high risk for renal failure and cardiovascular disease even in the general population seems to be much more than an appealing perspective.
